The
Personal Service Agreement (often
referred to as a PSA or Form
CO-802A )
is a contract for professional or technical services between
the University and individuals, partnerships or corporations. These
contractors are not employees of the State, but are hired for a
fee to provide services that are infrequent and unique, and primarily
noncompetitive in nature.

Follow these simple steps to completing or amending your PSA.
If you are contracting with an "International Visitor"
please call Accounts Payable @ 486-4137 to determine what
documentation must be completed prior to beginning the PSA process.
Using Foundation Funds to pay your PSA? University
accounts must be used and then reimbursed by Foundation Funds.
Foundation funds cannot pay a PSA contractor directly on the
University's behalf. Foundation questions call Meg Smith @ 486-2006.
